You'll have to look up "Matrix II" on Google. It normally costs money, but some sites have it for free. I can't supply it myself due to legal issues.
Also, I don't suggest using these blanks. They're old, small, and not particularly accurate. I have some more-accurate and much larger blanks elsewhere in my gallery, though they're for classic Japanese or Asian-English cards.
Download the right fonts, open the images in an image manipulation program (like Photoshop or GIMP), draw text boxes and type.

To be honest, when I made these, I don't think I even knew about token cards. Back when I played, we always used coins or something as tokens. You could probably paste the Dark Synchro template on the Synchro template at 50% opacity or so to get what you need.

BTW, man, do you have the *.psd? I'd REALLY appreciate if you could make (all) those templates (even the XYZ one) with the old OCG layout? That one with the smaller text box and the ATK/DEF boxes by the right...
